The difference wouldn't be defined by whats not in it but by what is. Cake flour is higher in wheat starch, you can use bread flour by replacing 25% with corn starch. 12 oz flour + 4 oz corn starch gives the same result as cake flour.

Pastry flour is between bread flour and cake flour, you can approximate it by mixing the 2.
